The 15% membership discount does not apply to the following:
Cosmetic treatments such as cosmetic bonding and porcelain veneers
Removable prosthodontics (complete and partial removable and non-removable)
Implant services
Fixed prosthodontics such as bridges
Orthodontics (comprehensive orthodontic treatment and appliance therapy)
Adjunctive general services such as oral conscious sedation
Procedures completed by outside specialists
When a current patient refers a new patient to the practice, both receive a $50 credit to their account — or, if you prefer, a movie package with two tickets, two drinks, and popcorn. The new patient must complete a comprehensive exam and dental cleaning. Credit can be applied toward any treatment or product in the office.

You may cancel and receive a full refund up to 30 days after purchase, only if no benefits have been used. If any benefit has been used, there are no refunds.
Payment for treatment is due at time of service. For certain procedures, a deposit is required to reserve appointment time.
Our 48-hour cancellation policy is strictly enforced. Should you 'no-show' or cancel outside of 48 hours, preventive appointments may be voided from your plan.
Using third-party financing such as CareCredit or Greensky voids the 15% discount on treatment plans.
This plan cannot be combined with dental insurance.
